Lakeland Prep Driver’s Permit Attendance Form

One of the requirements to receive a Tennessee Learner's Permit is to show proof of school attendance from your school.  

Students must meet the following requirements:

• Must be 15 years old or older (or 1 week before 15th birthday)
• Have fewer than 15 unexcused absences
• Have fewer than 10 consecutive absences
• Must be enrolled in school

Students meeting these requirements may request a form in the high school main office.  Forms requested by Friday will be available for pickup the following Tuesday during lunch or after school.  Certificates are valid for 30 days.  If the certificate expires and no permit is obtained, the student must request a new form from the main office
